I like the trip idea for her and her sister. otherwise group party for the two of them would be good, do it a week before or after their birthdays in order to surprise them. we had a surprise party for my moms 60th at a local restaurant party room with like 30 guests. it was great, i made a picture board of photos of her throughout the years and got her a Life magazine from the week she was born on Ebay. I had the tables decorated with lilies in a vase and fake pink rose petals down the center of the table with some purple candles, pink and purple balloon arrangements and streamers everywhere. we wanted to do more elegant birthday rather than tacky over the hill crap. she was very surprised, we just told her we were taking her to dinner and everyone was there waiting for us! our party was $1000 total which included the hall, food (we had chicken kiev, mashed potatoes, salad, and steamed veggies), and some wine and beer for each table.

Could you and your cousins organize a surprise party for the two of them? Perhaps your mom's twin's kids could get her to your house but they don't have to tell her about the party. You could invite your other family members and friends and SURPRISE! 50 is a pretty big deal - I'd go all out!
