Chain comes loose about 10 seconds into cutting every time

I've tried everything. New chain' tightening it by pulling the bar up like everyone says. Bar doesn't seem to be loose when I take it back apart to tighten it and the tightening scew thing isn't loosening either. Worked fine when I got it then started acting up after I used it on some roots in the ground. I cleaned it all up.
